How do you fill out Form A?

In the participant form we ask candidates to be exhaustive, to write as many details as possible regarding the questions asked: as you can imagine, we need this information to guarantee the safety of all participants, and to be able to structure a program that is as interesting as possible for the our guests.

Who needs to fill out Form B?

Form B must be completed by the participant’s specialist doctor. If you think you will not be able to see or have contact with your specialist doctor in a timely manner, it is fine for your GP to fill it out.

How do you fill out form B?

Form B must be delivered to the specialist doctor by the candidate himself, and the doctor can give it to the candidate (completed, stamped and signed) to send to Campo Base, or the doctor himself can send it to info@associazionecampobase.org
If the specialist doctor is not available in due time, it is possible to have the GP complete the form.
Please inform the doctor who fills out the form who may be contacted if necessary for any clarifications once the forms have been received.
For safety reasons, if the candidate has ongoing or recently completed treatments or other particular conditions, we may request that you carry out a blood test or other laboratory test that is more up to date or close to the event.
If it is not possible to send the Health Form B quickly, please send the Participant Form A first, and then send the health form as soon as possible.

Do I have to undergo specialist visits or tests to participate?

For safety reasons, if the candidate has ongoing or recently completed treatments or other particular conditions, we may request that you carry out a blood test or other laboratory test that is more up to date or close to the event.
It is assumed that when filling out the form the candidate will consult with his or her doctor.

If I send the forms will I definitely participate in Way Up?
Submitting the form does not guarantee participation in the project. The association reserves the right to draw up the list of confirmed candidates following the criteria defined by it. For information regarding this, contact us via email at info@associazionecampobase.org
Confirmation of participation or non-participation will arrive following the delivery of the medical form.
The chronological order of receipt of the forms constitutes one of the factors with which the Campo Base Association staff defines the list of confirmed participants, therefore we ask you to return the completed forms as soon as possible.

By when do I have to send the modules back to Campo Base and how?

The forms must be sent to info@associazionecampobase.org by JUNE 15th, 2024.

The chronological order of receipt of the forms constitutes one of the factors with which the Campo Base staff defines the list of confirmed participants, therefore we invite you to return the completed forms as soon as possible.
If it is not possible to send the Health Form B within the time limit, please send the Participant Form A first, and then send the health form as soon as possible.
